CK Supply Fuel Surcharge Prices
Due to increased diesel fuel prices, CK Supply is forced to implement an additional fuel surcharge on invoices in the amount that is specified below. This surcharge will be calculated on a monthly basis. Pricing is provided by the Department of Energy website, Midwest.

Price Range Surcharge
- At or below $3.614 / Gallon = $0.00
- $3.615 – $3.99 / Gallon = $6.00
- $4.00 – $4.49 / Gallon = $7.50
- $4.50 – $4.99 / Gallon = $9.00
- $5.00 – $5.49 / Gallon = $10.50
- $5.50 – $5.99 / Gallon = $12.00
- $6.00 + / Gallon = $15.00
The added temporary fuel surcharge will apply to invoices where products are delivered to a customer’s site and will not apply to cylinder rental invoices.
